COVID Vaccination Scheduling
Directions regarding COVID -19 Vaccine:
Please review, print and complete ALL necessary documents before requesting to schedule your appointment via our functional email firstname.lastname@example.org.
DO NOT call our office, one of our schedulers will respond by email to schedule both appointments based on our clinic availability. If you qualify within the Vaccine Distribution Phases defined by CDPHE we will schedule you to receive the vaccination. Anyone receiving the vaccine will need a state identification card. Healthcare workers are required to bring a picture employment badge to be eligible as a Healthcare worker.
If you need to cancel your appointment please DO NOT cancel the appointment the same day. Vaccines are distributed to us in a 10 dose vial and they are only good for 6 hours once
the bottle is opened. If you cancel the same day it is likely we will need to waste a dose that could have been allocated to another person in need. On the day of your appointment you will wait in your vehicle until an Astra employee calls you and give you further directions.
